Delayed Ejaculation and alcohol abuse

My husband has suffered from delayed ejaculation for many years now from time to time. It's getting progressively worse. He's also a functional alcoholic (and has been for over 20 years), BUT he won't admit to either condition. Recently, he finally went to a medical professional to rule out diabetes and other physical causes, but neglected to tell the doctor about his sexual issues or about his alcohol abuse. Instead, he then convinced the doctor that he needed ambien because of his travel for his job. He's been using the ambien, instead, to boost his libido. He hasn't told me, but it's obvious as he's going through the pills here at home, finally able to ejaculate BUT is only able to participate in a zombie like sex where he is completely passive and gets manually or orally stimulated by me.

I'm seriously at my wits end. I can live without sex, if I have to, but this is much worse to me. We've been married for 27 years and I hate to give up on him and our marriage, but I'm more stressed than I've ever been in my life. With today's privacy laws, how can I get his physician the information he needs to actually help him?
