Going to Urologist- advice?

I finally got my referral to the urologist from my primary doc, so I go in 3 days. I dont know too much about varicoceles, just what I learned from this page & a couple other websites. My first action was going to tell the doc "I want the surgery, so I wont have any problems later on" (worse pain, not being able to have kids, etc) Would this be a smart move on my part?
If the doc tries to talk me out of the surgery- what are the side effects of living with a varicocele?(testicle keeps getting bigger, other one smaller? wont have kids for sure? constant pain coming & going?) If so, living w/ one doesnt sound too good to me.
I also read another post where someone's testicle was smaller due to the varicocele. My left one has the varicocele & its bigger than my right. I wonder why- but I definitely dont want to end up with uneven testicles. (no matter which one is bigger)
I havent had sex since I had the pain & kinda scared to orgasm now. Does the varicocele cause pain during or after sex? (if so, I dont want to live w/ that- it will turn me off from sex)
Is there any things I definitely need to find out or ask before I leave the urologist office?
I dont want him to just tell me " Oh, you'll be fine" & send me on my way.
